Vacancies x 4: Social Prescribing Service for Voluntary Centre Services

Voluntary Centre Services is a charitable company that delivers volunteering development and voluntary sector development support services across West Lincolnshire. Voluntary Centre Services has four exciting opportunities for highly motivated individuals to join a small team as part of our Social Prescribing Service.

The Social Prescribing Service, developed alongside the Integrated Neighbourhood Teams (INT’s) in West Lincolnshire provides a means of enabling primary care services to refer patients with social, emotional or practical needs to a range of local, non-clinical services.

Our new team members will support the development of Social Prescribing in Lincoln North and Lincoln South, building on the success of the Gainsborough pilot project.

Social Prescribing Navigators will be responsible for delivering a person-centred service, supporting people to improve their health and wellbeing by connecting with services in their community. Navigators will be a vital community resource, responsible for patient engagement and developing effective working relationships with health care professionals and third sector providers.

Project Support Officers will provide dedicated support to the development of the Social Prescribing service, act as a point of contact for enquiries and referrals, support with marketing and communications and provide administrative support for the office.

Post 1- Social Prescribing Navigator (Part time – Sleaford based)

£15,763 per annum (£22,518 FTE) / 24.5 hours per week / Sleaford based

£15,763 per annum (£22,518 FTE) / 24.5 hours per week / Lincoln based
